What are Care Management Services?

A range of services designed to help preserve and maximize client independence.

  • RN Care Management (Assessment, consultation and monitoring by a licensed Registered Nurse).
  • Health Watch™ (a personal emergency response system).
  • Bill Management services (including assistance with bill organization and payment, reconciling financial records, mail pickup and delivery).
  • M.D. 2 Personal Medication System.

Who can benefit from Care Management Services?

  • People who need assistance identifying choices, options, resources and services available in the community.
  • Families who can no longer juggle their own responsibilities and the needs of their relatives.
  • Client representatives or medical professionals concerned about the care needs of an older or disabled adult.

How can I benefit from Care Management Services?

  • Client's needs assessed and individual recommendations made to assist a person to continue to live safely and independently, either in their own home or in an alternative setting.
  • Changes evaluated and monitored and significant others informed.
  • Assistance with keeping and reconciling financial records as well as organizing and paying bills.
  • Advocating for you with your insurance company.
